Polished rod coupling came up through the stuffing box.
Polished rod was rubbing against bottom of BOP.
The bottom of BOP ram hit by polished rod coupling.
Stuffing box ram screw is bent.
Caused by chemical reactions, CO2 and H2S.
Caused by a number of reasons including erosion corrosion, caused by H2S and C02.
A build up of paraffin which can create a situation where the flapper will not close.
The rams were opened with more than 1000 pounds of pressure without equalization.
The flapper was damaged due to installation of polished rod without a bullet.
